What does that mean?
The Patient-Centered Medical Home (PCMH) is a concept that is trying to change the face of primary care. By practicing team-based care in the office and fostering a true partnership between patient and provider, the PCMH is hoping to help improve patients’ health and help reduce much of wasteful spending in American healthcare system.
FMGW has been working on implementing this model of care in our office. By following the seven principals in the PCMH (listed below), we strive to provide you with the best healthcare possible.
Seven Principals of the PCMH
Personal Physician ~ having one doctor that coordinates all of your healthcare
Physician directed medical practice ~ having a trained doctor over-seeing the office
Whole person orientation ~ treating the entire person, not just one organ or system in the body
Care is coordinated and/or integrated ~ multiple providers (i.e. nutritionists, psychologist…) coordinate patient care in a team based approach
Quality and safety ~ reducing medical errors and providing evidence based care
Enhanced access ~ not having long wait times to get in to see a provider
Payment ~ making the concept financially viable

Practice Administrator's View on Medical Home
As the Practice Administrator of Family Medicine, Geriatrics &Wellness, I take my role very seriously. I strive to make the office work well for the patients, as well as provide job satisfaction to the employees and the providers.
What does this mean to me as an employee of Family Medicine, Geriatrics & Wellness?
It means alot. It means that everyone here, whether in direct support or indirect support of patient care, matters tremendously. We are a team, a unit that puts the patient first. We should all know our patients, their family, and together we all manage their total health with them. It also means that everyday, each and every one of us, makes a difference in the life of our patients. Think about that. We make a difference in our patient’s health and well being. This is a privilege we all need to take very seriously. We often see our patient’s when they are most vulnerable. But the reality is that we are with our patient’s all the time. When they are well, they have the peace of mind that an entire group of people care for them. From taking phone calls and blood pressures, to billing their insurance companies and scanning in their personal records; each and every one of us should be very proud of what we do, I know I am. Because we are part of the Patient Centered Medical Home, our job goes far beyond that of other medical offices. I am proud of that.
I hope to serve as a model to this approach, not only in my words but in my actions. I take patient care very seriously and I believe every patient deserves to be treated with the upmost respect, TLC, and support.
Being part of the Patient Centered Medical Home Model does mean going above and beyond, and I’m very glad to see that in my time here so far, my employees live that philosophy. It is my job to make sure that continues by providing them with all the tools possible. I strive to make this a happy, healthy, work environment where the patient’s and the employees matter most.
I am here five days a week. My door is always open. My e-mail is readily accessible. And if you know our office, I take every phone call given to me, as well as answering the calls throughout the day. It is my pleasure to welcome you and to thank you.
